About the Role:
Royal Crown Homes LLC is looking for a dedicated and compassionate Designated Coordinator to join our team in Bloomington, MN. In this vital role, you'll be at the heart of supporting individuals with disabilities, ensuring they receive the highest quality of care and services. Responsibilities:

• Develop, implement, and monitor individualized service plans (ISPs) for individuals receiving residential support services
• Serve as the primary point of contact for individuals, families, case managers, and other stakeholders
• Coordinate and facilitate person-centered planning meetings and reviews
• Ensure compliance with DHS licensing standards, 245D regulations, and all applicable state and federal guidelines
• Supervise and support direct support professionals (DSPs) to ensure quality care delivery
• Maintain accurate documentation, progress notes, and service records
• Conduct assessments and identify support needs, goals, and outcomes for each individual served

Requirements:

• Experience working in residential services, disability services, or a related human services field
• Knowledge of Minnesota 245D licensing requirements and DHS regulations
• Designated Coordinator qualifications as defined by Minnesota Statute
• Strong person-centered planning and individualized service plan (ISP) development skills
• Excellent communication, organizational, and documentation skills
• Ability to pass a background study through the Minnesota Department of Human Services
• Compassionate, dependable, and committed to advocating for individuals with disabilities
