Position Overview:
As a Diabetes Educator, you will play a fundamental role in helping hospitalized patients gain the knowledge, skills, and confidence needed to successfully manage their diabetes. Working collaboratively with providers, nurses, dietitians, and care teams, you will provide individualized education on blood glucose monitoring, medications, insulin administration, nutrition, lifestyle modifications, and diabetes technology. You will assess each patient's unique needs, develop personalized education plans, support discharge readiness, and help ensure a smooth transition to outpatient care. This role offers the opportunity to make a meaningful impact on patient outcomes by empowering individuals to take an active role in managing their health.
