Job Description

Overview:

Every patient’s diabetes journey is different. Some manage their care independently, while others lean on family and caregivers for support. As a Diabetes Educator, you’ll meet each person where they are, using your clinical expertise, compassion, and personalized education to help patients and families feel confident managing their health. You’ll also have the autonomy to navigate complex diabetes management needs while collaborating closely with physicians, nurses, and other members of the care team.

If you’re looking for a role where you can practice independently, solve complex clinical challenges, and be part of a supportive team, we’d love for you to join us as a Diabetes Educator.

Why Legacy Salmon Creek

At Legacy Salmon Creek Medical Center, you’ll bring your diabetes expertise to a modern, collaborative environment where your knowledge and perspective are valued. You’ll join a close-knit diabetes education team and a supportive community of Certified Diabetes Care and Education Specialists (CDCES) across the Legacy system.

You’ll have the independence to use your clinical judgment and manage your work while knowing you have experienced colleagues to turn to when a challenging case calls for another perspective. With strong clinical partnerships, accessible leadership, and a connected CDCES community, you’ll experience a rewarding balance of autonomy, collaboration, and support.

Responsibilities:

What You’ll Do

Make a difference for patients and families

Every patient brings a different story and different needs. You’ll work with a diverse patient population through surgery, critical care, obstetrics, and other inpatient settings, helping individuals and families navigate a wide range of diabetes management needs.

  • Bring your expertise to a variety of patient needs, from new diabetes diagnoses and insulin pump management to steroid-induced hyperglycemia and other complex situations.
  • Build confidence through personalized education, assessing each patient’s needs and creating an approach that meets them where they are.
  • Use your clinical judgment and problem-solving skills to identify education needs and help patients navigate complex diabetes management challenges.
  • Partner with patients and families to build the knowledge and skills they need to manage diabetes beyond their hospital stay.
  • Collaborate across the care team to support treatment, education, and discharge planning.

Help shape diabetes care

Your impact won’t stop with individual patients. You’ll have opportunities to share your perspective and help strengthen diabetes education and care across Legacy.

  • Partner with your colleagues to develop education materials, programs, and resources.
  • Help shape system-wide standards and best practices for diabetes education.
  • Bring forward ideas that improve the patient’s experience and effectiveness of diabetes education.
  • Use evidence-based practices and continuous improvement principles to help advance the program.

Be the diabetes expert others turn to

Your expertise will also make a difference for the clinicians around you.

  • Serve as a trusted resource for nurses, physicians, and other members of the care team.
  • Share your diabetes expertise through staff education and interdisciplinary learning.
  • Give colleagues from other disciplines opportunities to shadow you and better understand diabetes education and management.
  • Extend your impact through community and professional education opportunities.

Keep growing your expertise

You’ll be encouraged to continue learning, stay connected with other diabetes educators, and bring new ideas back to your team.

  • Stay current on emerging research and best practices in diabetes care and education.
  • Connect with diabetes educators across Legacy to exchange ideas and learn from one another.
  • Continue developing your expertise through ongoing education.
  • Participate in research opportunities when appropriate.

Ready to Make a Difference?

If you’re looking for a role where you can practice with independence, take on meaningful clinical challenges, and have a strong team behind you, we’d love to hear from you.
