Responsibilities:The Coordinator is a member of both the Diabetes Management Team and the Institute for Cancer Care and serves as the resource and instructor for patients, families and professionals on nutritional issues in the management of diabetes and cancer. The Diabetes Education Program Coordinator oversees the planning, implementation and evaluation of the DSMES services. The Coordinator demonstrates knowledge of diabetes, bariatrics surgery, oncology, lipid and, eating disorders. This is in addition to, principles of growth and development appropriate to the ages of the patients served. The Coordinator assesses the patient, provides dietary recommendations, instructs the patient/family in the plan and evaluates the success of the patient and plan to meet metabolic goals of the patient.
