Direct Care Monitor

We are seeking a committed Direct Care Monitor to support individuals experiencing mental health challenges, substance abuse issues, and homelessness. This position provides essential assistance and resources.

Responsibilities:

• Deliver support services, including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, and connection to community providers.

• Ensure the safety of residents and the facility.

Qualifications:

• Demonstrated commitment to assisting high-acuity individuals.

• Minimum of 1 year of case management experience or related experience.

• Experience with HMIS is a plus.

Tuesday-Saturday Swing 3pm -11:30om | Onsite

Pay Details: $21.00 to $24.00 per hour
