About the Role
We are seeking an experienced, dynamic, and results-driven professional to join our executive leadership team as the Director of Coordination. In this critical leadership role, you will oversee the daily operational execution of our home care coordination and staffing infrastructure. You will be responsible for ensuring seamless, high-quality, and person-centered care delivery to our clients by leading a team of Care Coordinators, Schedulers, and Intake Specialists.
The ideal candidate possesses deep expertise in home care regulations, scheduling optimization, caregiver retention, and workforce logistics. As the Director of Coordination, you will bridge the gap between clinical requirements, field staff capabilities, and client preferences to maximize operational efficiency, maintain regulatory compliance, and drive overall organizational growth.
Core Responsibilities
1. Strategic Staffing & Resource Allocation
- Architect, implement, and maintain strategic staffing processes to ensure 100% shift fulfillment across private duty, Medicaid, and complex clinical care cases.
- Monitor regional census fluctuations and forecast staffing demands to proactively source, onboard, and deploy Home Health Aides (HHAs), Certified Nursing Assistants (CNAs), and Registered Nurses (RNs).
- Manage on-call rotations, after-hours protocols, and contingency staffing plans to address emergency call-offs, hospital discharges, and rapid client admissions.
- Analyze caseload distributions to avoid caregiver burnout, minimize unbilled overtime, and optimize travel routes and labor utilization.
2. Home Care Coordination & Service Delivery
- Direct the coordination pipeline from initial client inquiry and assessment through ongoing care plan execution and service termination.
- Ensure precise matching of caregivers to clients based on clinical acuity, personality alignment, geographic proximity, and language preferences.
- Serve as the operational liaison between clients, family members, case managers, managed care organizations (MCOs), and interdisciplinary clinical teams.
- Establish and monitor Key Performance Indicators (KPIs) related to coordination, such as referral-to-admission turnaround time, fill rates, late arrivals, and unstaffed hours.
3. Team Leadership, Coaching & Development
- Lead, mentor, and evaluate a dedicated team of Staffing Coordinators, Intake Coordinators, and Administrative Support Personnel.
- Design and conduct comprehensive onboarding and training programs for coordination personnel focusing on customer service excellence, software mastery, and crisis de-escalation.
- Conduct regular performance evaluations, establish measurable quarterly benchmarks, and design corrective action plans when performance deviations occur.
- Cultivate a supportive, accountable, and high-energy culture that fosters talent retention and operational precision.
4. Regulatory Compliance & Quality Assurance
- Uphold stringent compliance with federal, state, and local home care licensing standards, as well as accrediting bodies (such as Joint Commission or CHAP).
- Audit Electronic Visit Verification (EVV) compliance metrics daily to ensure billing accuracy, combat fraudulent sign-ins, and avoid claim denials.
- Ensure all field staff maintain up-to-date credentials, certifications, medical health screenings (PPD, physicals), and background checks prior to and during client assignments.
- Partner with the Director of Nursing and Quality Assurance committee to review incident reports, client complaints, and implement targeted Corrective Action Plans (CAP).
5. Operational Systems & Technology Optimization
- Leverage Home Care Electronic Medical Records (EMR) and Enterprise Scheduling Software (e.g., HHAeXchange, ClearCare/WellSky, AlayaCare) to its fullest operational capacity.
- Identify operational bottlenecks and integrate automated notifications, shift broadcasting tools, and mobile workforce platforms to streamline coordination workflows.
- Generate and present weekly executive reports detailing staffing ratios, hours authorized versus hours delivered, churn rates, and profit margin analysis per case.
Qualifications and Experience
Required Qualifications:
- Bachelor’s Degree in Healthcare Administration, Business Administration, Nursing, Human Resources, or a closely related field (Master’s preferred).
- Minimum of 5 to 7 years of progressive operational experience within a Licensed Home Care Services Agency (LHCSA), certified agency, or private duty home care setting.
- Minimum of 3 years in a direct supervisory or managerial capacity overseeing staffing departments, coordination units, or high-volume healthcare logistics teams.
- Demonstrated expertise with Electronic Visit Verification (EVV) systems and leading home care platforms (such as HHAeXchange, WellSky/ClearCare, MatrixCare, or KanTime).
- In-depth comprehension of state department of health regulations, Medicaid Managed Long-Term Care (MLTC), Veterans Affairs (VA) programs, and commercial insurance coverage policies.
Preferred Competencies & Skills:
- Exceptional conflict resolution and crisis management skills; able to remain composed and make sound operational decisions in high-pressure scenarios.
- Data-driven mindset with strong analytical ability to translate staffing data and scheduling metrics into actionable business strategies.
- Superb interpersonal communication skills, with an empathetic approach to handling vulnerable clients, anxious families, and diverse field personnel.
- Bilingual proficiency (English/Spanish or other regional languages) is considered a strong asset.
