    POSITION SUMMARY

    Responsible for providing comprehensive nursing care and services to the residents as well as supervision of the nursing assistant staff of at Twin Lakes Community.

    Essential Duties and Responsibilities:

    • Responsible for maintaining RN licensure.
    • Responsible for initiation and implementation of comprehensive nursing care to residents. Responds or follow up on Emergency Calls and assists in Nursing Clinic as required.
    • Insures initial assessment of resident needs, care plan development, and appropriate follow-up assessments are accomplished. Must be available for consultation as necessary.
    • Monitors any significant changes in residents' status and ensures proper documentation and communication with healthcare team.
    • Performs ongoing review and updates of each residents' care plan and medication list.
    • Performs weekly pill box service and collaborates with Pharmacare Pharmacy for medication management services for residents.
    • Collaborates with Medical Director and Nurse Practitioner on a regular basis to provide care to their patients.
    • Understands nursing assistant scope of practice and communicates with RA/NA team daily to ensure the best continuity and quality of care.
    • Collaborates with the Administrator in hiring, orienting, corrective action and, if necessary, termination of nursing assistant staff.
    • Participates in Resident Care Reviews to identify changes and/or concerns.
    • Responsible for assessing resident need for higher level of care and directing transition as necessary.
    • Collaborates with various TLC team and community professionals (ex: hospital case manager) to determine residents' safety and ability to meet current care needs in assigned neighborhood following a decline in resident condition.
    • Monitors accuracy and completeness of physician's orders and the notification of physicians regarding changes in residents' conditions.
    • Provides counseling to residents' and families as needed and refers specific problems to the appropriate TLC employees as necessary.
    • Completes all necessary documentation on current residents and ensures all proper documentation is either sent or retrieved if resident moves from one level of care to another.
    • Responsible for protecting any confidential information concerning employees, residents, family, and/or the organization.
    • Must demonstrate knowledge of and adhere to all Safety procedures and facility Fire, Disaster, and Bomb Threat procedures.
    • Adheres to all organizational, departmental, and nursing procedures in the performance of position responsibilities.
    • Must attend organizational or departmental conferences and/or in services as scheduled.
    • Possess the ability to make independent decisions when circumstances warrant it using good judgement and ethical conduct.
    • Able to perform job duties and work independently with frequent interruptions.
    • Able to organize, prioritize, and manage time, multiple tasks and projects
    • Able to meet the physical demands and working environment of the position.
    • Able to follow procedures.

    Education and/or Experience

    • Five or more years of experience as a Nursing Supervisor/ADON in a patient care setting preferred
    • Associate Degree Required

    Certificates/Licenses Required

    • Valid registration as a Registered Nurse in NC.
    • CPR certification from the American Heart Association
