Directs, plans, organizes, and evaluates the Social Work / Care Management staff, daily operations, services and programs. Oversees comprehensive, culturally competent, psychosocial coordination of care. Monitors and manages patient care to promote continuity of care, optimal patient outcomes, and patient satisfaction. Assists a team of social workers and coordinators who work closely with physician-led multidisciplinary care team regularly to evaluate patient’s status and appropriateness of rehabilitation care, including admission, length of stay, transfers and discharge Ensures necessary resources for patients and families are identified in the community, and builds necessary relationships within the community to assist in transitions of care.

WORK PERFORMED:

• Leads the Social Work / Care management department by developing, communicating, and building consensus for goals/programs that support Burke’s business operations and patient needs.
• Oversees development, implementation, administration, and evaluation of Social Work / Care Management department.
• Oversees daily operations to optimize operational services, monitors operations, evaluates performance and implements improvement plans (PI).
• Establishes policies and procedures for the efficient, effective and professional operation of the department; selects, develops, manages, and evaluates direct-reports.
• Directs the fiscal management by participation in planning budget, space and personnel in such ways as to ensure appropriate and adequate services in line with stated goals and objectives.
• Maintains assurance of high quality care within the care management program and adequate documentation of services given. Conducts random reviews of social work documentation to assure proper utilization of services in order to meet patient needs, organizations goals.
• Monitors professional and clinical practice and provides applicable education and training to social workers and facility personnel; ensures professional credentialing of all licensed staff on an ongoing basis.
• Participates in Hospital administrative functions as representative of Care Management Department and communicates needs and ideals of department to administration as well as the concerns of administration to Social Work/Care Management staff.
• Attends the daily huddle and reports on pending discharges from Burke.
• Directs the Utilization review process for patients identified by the Social Work Care Managers or denied by insurance.
• Maintains liaison with other hospitals and community agencies in reference to marketing Burke's services.
• Serves as a social work resource and provides leadership and guidance to the social work/care management staff as well as the interdisciplinary team as needed. Actively participates in unit-based efficiency meetings.
• Coordinates weekday and weekend social work care management coverage.
• Monitors and documents the process of employee performance evaluations and competencies, reviewing skills and practices, assessing training and implementing training programs.    
• Promotes communication and coordination between social work/care management staff and other programs and departments within the organization.
• Actively participate in community education and serves as a care manager/social work resource for referral sources; takes part in community activities and wants sponsored by health related programs.
• Assists in covering caseload for other social workers’/care managers in the hospital as needed.
• Demonstrates personal adherence to the Corporate Compliance Program and related activities.

• Practices Standard Precautions. Follows regulations, policies and guidelines on the prevention of infections; does not eat or drink in patient care areas.
• Maintains confidentiality of information; abides by HIPAA regulations.
• Maintains a professional image and follows Hospital’s policy on uniforms; consistently wears hospital ID.
• Demonstrates reliable, on-time attendance, which is an essential function of all jobs at Burke.
• Completes mandatory annual learning (Learning Harbor) within deadline.

JOB SPECIFICATIONS:

Education & Experience Required:  LMSW, LCSW preferred.  Eight years’ experience including five years of supervision and management experience.  IRF experience preferred. Certified Case Manager.

 

Knowledge, Skills & Abilities:   Regular and significant interaction with all levels of management, including executive, in planning and problem solving activities.  Effective oral and written communication skills and ability to work with all administrative, medical, nursing, clinical, finance and support areas.  Must develop relationship with referral sources as well as maintain relationships with community agencies.

 

Work Environment: Both office setting and meetings either in Hospital or in community.  Work flow often interrupted by emergencies and crisis situations, and constant distractions presented for quick problem solving.  Necessary to maintain flexibility to deal with myriad of situations.  No closure to work.
