At Glacier Hills, we believe every resident deserves to feel supported, valued, and heard. We're looking for a compassionate Social Worker to join our team and make a meaningful difference in the lives of the older adults we serve within our Skilled Nursing community.

In this role, you'll build trusting relationships with residents and families, helping them navigate important transitions and providing support when it matters most. • Assist with resident assessments, care planning, and discharge planning.
  • Conduct cognitive and mood evaluations and coordinate follow-up services as needed.
