    Little Masters Development Center is an established Early Intervention agency serving children and families throughout all five boroughs of New York City. We are committed to providing families with compassionate, reliable, and high-quality support throughout their Early Intervention journey.

    We are currently seeking an experienced Early Intervention Service Coordinator to join our team in a hybrid role, with a combination of in-office and remote work.

    Position Responsibilities

    The Service Coordinator will be responsible for supporting infants, toddlers, and their families throughout the Early Intervention process. Responsibilities include, but are not limited to:

    • Ensure that Early Intervention services are provided in a timely and coordinated manner.

    • Inform families of their rights, procedural safeguards, and available services.

    • Coordinate funding sources and assist families with accessing appropriate resources.

    • Process referrals, schedule initial meetings with families, and conduct screenings using standardized protocols and tools approved by the New York State Department of Health (NYSDOH) and Bureau of Early Intervention (BEI).

    • Coordinate, schedule, and attend evaluations and assessments.

    • Coordinate and participate in IFSP meetings, including quarterly, six-month, and annual reviews, as well as eligibility determination and transition planning meetings.

    • Coordinate with families, providers, evaluators, and other involved parties and ensure that all parties are notified of meeting dates and times.

    • Maintain thorough and accurate documentation in accordance with applicable state regulations and Early Intervention requirements.

    • Assist eligible children and their families in accessing Early Intervention services and other supports identified in the Individualized Family Service Plan (IFSP).

    • Provide ongoing service coordination and support to families of infants and toddlers with developmental delays.

    • Connect families with community resources and provide guidance and support related to child development and available services.

    • Assist families in navigating the Early Intervention system and advocating for appropriate services.

    Position Requirements

    • Prior Service Coordination and/or Early Intervention experience is required.

    • Demonstrated knowledge of the Early Intervention system, including IFSPs, evaluations, eligibility determination, service planning, and transition procedures.

    • Strong organizational, communication, documentation, and interpersonal skills.

    • Ability to effectively manage multiple cases, deadlines, and documentation requirements.

    • Ability to work collaboratively with families, service providers, evaluators, and other professionals.

    • Bilingual Service Coordinators are strongly encouraged to apply.

    • Must be dependable, professional, organized, and committed to supporting children and families.

    Work Schedule

    This is a hybrid position. Candidates must be available to work in person at our Kew Gardens, Queens office as needed, based on operational and business requirements.

    Compensation

    $12–$14 per unit, based on experience and qualifications.

    This position is compensated on a per-unit basis. The compensation amount listed in the CareerPlug salary field is for posting purposes only and does not represent an annual salary or hourly rate.
