A healthcare organization is seeking a compassionate and patient-focused Registered Nurse (RN) to support specialty outpatient services and provide high-quality care to patients managing chronic health conditions. This opportunity is ideal for nursing professionals who enjoy patient education, care coordination, and building meaningful relationships with patients throughout their healthcare journey.
Conduct patient assessments and assist with ongoing care management
Administer medications, injections, and prescribed treatments
Educate patients on disease management, treatment plans, medications, and self-care strategies
Provide guidance regarding monitoring devices, therapies, and preventive care measures
Respond to patient inquiries and assist with care coordination activities
Collaborate with physicians and interdisciplinary healthcare teams to support patient care plans
Document patient interactions and clinical information accurately and timely
Promote infection prevention and patient safety standards
Support patients managing chronic medical conditions through education and follow-up care
Contribute to a positive and collaborative team environment
Current Registered Nurse (RN) license or eligibility for licensure
Graduate of an accredited nursing program
Current Basic Life Support (BLS) certification
Previous outpatient or ambulatory nursing experience preferred
Experience providing patient education preferred
Strong communication, assessment, and organizational skills
Ability to work effectively in a team-oriented healthcare environment
