We are currently in search of a Clinical Coordinator for a Housing-First Nonprofit. The position is on-site with openings in across the 5 boroughs. This is a full-time, permanent position.

LOCATION: New York City

POSITION TITLE: Clinical Coordinator

SCHEDULE: Mon-Fri 9am-5pm (35 hours/week)

DURATION: Permanent

SALARY: $64,000 - $68,000

 

Is This Your Dream Job? A housing-first nonprofit is searching for a Clinical Coordinator to help people experiencing chronic homelessness gain housing stability.

 

The Clinical Coordinator manages and supervises a team of Case Managers, ensuring the utilization of Harm Reduction and Housing First strategies and other best-practice clinical interventions to ensure the highest quality care for clients. This role is also responsible for maintaining client records in compliance with regulatory and agency standards.

 

There are openings at multiple locations throughout Brooklyn, Queens, Manhattan, Staten Island and the Bronx. All roles are day-time hours with availability to be on-call for emergencies.

 

Our Ideal Candidate:

  • NYS Licensed Masters Social Worker required (LMSW)
  • 3+ years of experience working with related populated
  • Previous supervisory or administrative experience preferred
  • Understanding of Housing First, client-centered motivational interviewing, Harm reduction models, behavioral and medical health systems
  • Experience with crisis intervention, including risk assessments and incident management
  • Able to delegate and motivate staff to achieve deliverables
  • MS Office Suite proficiency required
  • Bilingual in English/Spanish a plus

 

How You Will Spend Your Day:

  • Supervise a team of Case Managers.
  • Provide regular, structured supervision meetings for direct reports
  • Promote effective strategies for case management following Housing First, Harm Reduction, Crisis Prevention Institute (CPI), Motivational Interviewing and trauma-informed care models of practice
  • Develop and implement individual service plans; ensure documentation of client contact and progress is comprehensive and timely
  • Interview and assess potential clients
  • Provide clinical direction and leadership to the program, help troubleshoot, problem solve, and strategize solutions around client engagement and challenging behaviors
  • Promote a staff and resident culture that emphasizes housing placement as a measurement of success; ensure that clients are ready for housing placement
  • Oversee development and implementation of individual service plans; ensure documentation of client contact and progress is comprehensive and timely
  • Assess and evaluate client functioning
  • Supervise and conduct case conferences
  • Coordinate delivery of care with multiple service providers, particularly outreach teams.
  • Work collaboratively with outreach teams to ensure individuals succeed in transitional housing and avoid return to homelessness.
  • Provide support and guidance to front line staff in managing difficult situations and successfully de-escalating conflict
  • Perform other duties as assigned
