Responsibilities*Assess and monitor hospice patients. *Provide skilled nursing care and symptom management.*Administer medications and treatments as ordered*Educate and support patients and caregivers.*Communicate changes in condition to the care team and physician * Maintain accurate clinical documentation. *Collaborate with the hospice interdisciplinary team. *Promote patient comfort, dignity, and quality of life.
