Our Community Market Case Managers help clients navigate challenges, connect with essential resources, and develop meaningful goals toward greater stability and self reliance. We're looking for someone who can build strong relationships with clients while bringing accuracy, organization, and accountability to every part of their work.About the RoleAs a Community Market Case Manager, you'll provide comprehensive case management services to older adults who may need assistance navigating food access, benefits, housing, healthcare resources, financial stability, transportation, and other supportive services.You'll work directly with clients to assess their needs, develop individualized service plans, connect them with appropriate community resources, coordinate referrals, and provide ongoing support as they work toward greater stability, independence, and quality of life.This is a highly client facing and data driven role. You'll balance meaningful direct service with detailed case documentation, including maintaining accurate client records, tracking services and outcomes, and completing required documentation in HMIS and Salesforce within established timelines.This is a great opportunity for an experienced Community Market Case Manager who enjoys working with older adults, understands the unique needs and barriers seniors may experience, and is equally comfortable building relationships with clients and managing the data and documentation behind their services.The ideal person for this role is compassionate, patient, highly organized, detail oriented, comfortable working with data, and able to balance an active caseload with documentation, referrals, follow up, and changing client needs.This is a full time position requiring approximately 40 hours per week, Monday through Friday, 8:00am to 4:30pm. Occasional evening, weekend, event based, or on call work may be required depending on operational needs.What You'll DoConduct intake assessments to identify client needs, strengths, goals, and barriers to long term stabilityDevelop individualized action plans with clients focused on housing, employment, health, financial stability, food access, and other essential needsProvide ongoing case management by monitoring client progress, reassessing needs, evaluating outcomes, and adjusting service plans as neededConduct regular client assessments and follow up to ensure individuals and families are receiving appropriate supportConnect clients with community resources, including housing programs, mental health services, employment opportunities, food assistance, financial assistance, and other supportive servicesFacilitate referrals and follow up to help clients successfully access available resourcesProvide crisis support, safety planning, and connections to appropriate emergency resources when necessarySupport clients with financial literacy, budgeting, saving, debt management, and other skills that promote greater self relianceAssess client employment goals and collaborate with workforce partners to connect clients with job training and employment readiness resourcesAssist SNAP program participants with applications, required documentation, necessary data, and connections to nutrition education supportMaintain the Community Market by restocking shelves, maintaining cleanliness, keeping signage current, and ensuring the space remains welcoming and organizedMaintain accurate food inventory records and submit weekly or as needed food orders to ensure consistent availabilityMaintain accurate and comprehensive case notes documenting client assessments, services, referrals, progress, outcomes, and relevant correspondenceRecord required client services, referrals, recommendations, case notes, and program documentation in HMIS or Salesforce within one business day of client interactionMaintain accurate shared spreadsheets and program tracking tools within established timelinesReview applicable grant and program requirements to determine client eligibility for available services and assistanceComplete required forms, reports, and supporting documentation in accordance with established proceduresParticipate in internal and external case conferencing to coordinate services and identify solutions for complex client needsMaintain current knowledge of community resources, eligibility requirements, social services, and referral processesPrepare and submit required client success storiesRepresent The Just One Project at organization wide events, including participation in six mandatory Pop Up & Give events each yearWhat You BringBachelor's degree in Social Work, Human Services, or a related field, or an equivalent combination of education and relevant work experienceThree or more years of nonprofit experienceThree or more years of intensive case management experienceFluency in both English and Spanish, including verbal and written communicationHands on experience using HMIS for client records, case notes, services, referrals, and program documentationStrong data entry and documentation skills with the ability to maintain accurate, complete, and timely client recordsExperience tracking client services, progress, outcomes, and program metricsExperience managing an active caseload while consistently meeting documentation, reporting, and follow up deadlinesExperience working directly with diverse populations, including vulnerable or at risk individuals and familiesStrong assessment, problem solving, advocacy, and conflict resolution skillsAbility to build trust and rapport with clients while maintaining appropriate professional boundariesAn empathetic, nonjudgmental, and trauma informed approach to client engagementStrong organizational skills and attention to detail with the ability to successfully manage multiple prioritiesKnowledge of community resources and ability to connect clients with appropriate services and referralsStrong written and verbal communication skillsAbility to maintain confidentiality and appropriately handle sensitive client informationProficiency with Microsoft Office, including Word, Excel, PowerPoint, Outlook, and related workplace applicationsAbility to work collaboratively with school personnel, community partners, internal teams, and other service providersAbility to work in an active Community Market environment, including regularly standing, walking, reaching, stocking, and lifting or carrying up to 50 poundsA valid, unrestricted driver's license, reliable personal transportation, and ability to meet organizational insurability requirementsAbility to obtain and maintain a valid Nevada Food Handlers Card within 30 days of hireLegal authorization to work in the United StatesA commitment to teamwork, Salesforce or similar client management systemsFamiliarity with housing services, wraparound programs, and self sufficiency initiativesProven experience managing a caseload and delivering measurable
