Services for Operation HOME! may include, and are not limited to: intensive client centered trauma informed case management, housing and rental assistance, assistance with diversion from homelessness and emergency shelters by obtaining documentation, case management services, support services, access to resources, and after-care. In addition, case managers will assist with move-in assistance, rapid rehousing, transitional housing paired with rapid rehousing, and/or supportive housing and interaction with the provider partner network and the homeless community queue.
