Field RN Supervisor (LHCSA Experience)

We are looking for a dedicated Field Nurse Supervisor to join our team and play a key leadership role in supporting field nurses, maintaining clinical standards, and ensuring regulatory compliance. This position combines fieldwork in patients’ homes with office-based administrative oversight.

The ideal candidate will have experience in home care, specifically within a LHCSA, and possess strong clinical assessment and documentation skills. You’ll act as a liaison between field staff, patients, families, and the administrative team while ensuring seamless delivery of quality care.

Key Responsibilities:

• Supervise, support, and train a team of field nurses.
• Conduct initial assessments, supervisory visits, and re-assessments in patients’ homes.
• Ensure accurate, complete, and DOH-compliant clinical documentation.
• Collaborate with the team to maintain timely compliance paperwork.
• Serve as a liaison between field staff, patients, families, and administrative staff.

Qualifications:

• Current NYS Registered Nurse (RN) license.
• Minimum 2 years of home care experience, specifically within a LHCSA.
• Strong clinical assessment and documentation skills.
• Comfortable traveling throughout Brooklyn and Queens for home visits.
• Supervisory or leadership experience is a plus.
