Job Summary:

The Form Carpenter is responsible for constructing, installing, aligning, bracing, and dismantling wood, plywood, steel, and aluminum concrete forms for heavy civil and industrial construction projects. This position plays a critical role in ensuring concrete structures are built accurately according to project plans, specifications, and quality standards while maintaining a safe work environment.

  • Construct, install, align, brace, and strip concrete forms for footings, foundations, walls, columns, beams, slabs, bridges, and other concrete structures.
  • Read and interpret construction drawings, blueprints, specifications, and layout information.
  • Measure, cut, and assemble wood, plywood, and other forming materials using hand and power tools.
  • Set and verify form elevations, grades, alignment, and dimensions to ensure accurate concrete placement.
  • Install form ties, braces, blockouts, chamfers, embeds, and other forming components as required.
  • Work closely with superintendents, foremen, equipment operators, ironworkers, laborers, and concrete crews to complete daily work activities.
  • Assist with concrete placement operations as needed to help ensure forms remain properly aligned and secure during pours.
  • Strip, clean, repair, and prepare formwork materials for reuse.
  • Maintain a clean, organized, and safe work area in accordance with company safety policies and OSHA requirements.
  • Inspect completed formwork to verify compliance with project specifications, quality standards, and safety requirements.
  • Safely operate hand tools, power tools, and other construction equipment necessary to perform assigned duties.
  • Perform other duties as assigned.

Requirements and Education:

  • Minimum of 3-5 years of experience as a Form Carpenter or Concrete Carpenter on commercial, industrial, or heavy civil construction projects.
  • Demonstrated experience constructing and installing concrete formwork using wood, plywood, steel, and aluminum forming systems.
  • Experience with foundations, walls, columns, slabs, bridges, retaining walls, and other cast-in-place concrete structures.
  • Ability to read and interpret construction drawings, blueprints, specifications, and layout information.
  • Knowledge of concrete forming techniques, construction materials, and standard construction practices.
  • Ability to accurately measure, layout, and verify grades, elevations, dimensions, and alignment.
  • Proficiency in safely operating hand tools, power tools, and construction equipment used in formwork operations.
  • Ability to work effectively as part of a construction team while following direction from supervisors and foremen.
  • Strong attention to detail to ensure precise, high-quality workmanship.
  • Ability to perform physically demanding work, including lifting, climbing, kneeling, and working in varying weather conditions.
  • Ability to obtain and maintain security clearance to access secured facilities, including military bases, when required.
  • Strong commitment to workplace safety and compliance with OSHA and company safety policies.

  • Work is primarily conducted outdoors at construction sites.
  • Exposure to various weather conditions and potential hazards.
  • Physical demands include lifting, standing, and operating heavy machinery.
  • May require flexible work hours and occasional travel.
