Full-Time Physical Therapist (PT) Near Kansas CIty, MO
Connected Health Care is seeking a dedicated and efficient Physical Therapist (PT) for a full-time opportunity in Kansas CIty, MO. This position is with one of our valued healthcare partners and offers the chance to work in a dynamic home health setting where your expertise directly improves patient outcomes and quality of life.
We are looking for a clinician who thrives in a fast-paced environment, is highly organized, and excels in patient-centered care. Experience with home health processes and documentation is strongly preferred.
Job Responsibilities
Provide high-quality physical therapy services in a home health setting
Perform patient evaluations and develop individualized treatment plans
Complete Start of Care assessments and ongoing case management
Conduct OASIS assessments accurately and efficiently
Manage re-certifications and ensure timely documentation compliance
Collaborate with interdisciplinary teams to coordinate patient care
Educate patients and families on treatment plans and home exercises
Maintain accurate and timely documentation using Matrix Care EMR
Monitor patient progress and adjust treatment plans as needed
Education & Certification Requirements
Doctor of Physical Therapy (DPT) or equivalent from an accredited program
Active Physical Therapist license in Missouri and Kansas required
Current CPR/BLS certification
Minimum of 1 year of home health experience preferred
Strong knowledge of OASIS, case management, and re-certification processes
Proficiency with EMR systems with Matrix Care experience preferred

Experience with Start of Care assessments and home health workflows
Excellent communication and interpersonal skills
Ability to work independently and manage a patient caseload effectively
