Full Time Home Health & Hospice LPN Licensed Practical Nurse

Connected Health Care is partnering with a respected home health and hospice provider in the Boise, Idaho area to recruit a compassionate and dedicated Licensed Practical Nurse (LPN) for a Full Time Home Health & Hospice opportunity. Position Overview
As a Home Health & Hospice LPN, you will work alongside an interdisciplinary healthcare team to provide skilled nursing services to patients in their homes. This role requires excellent clinical judgment, strong communication skills, and a commitment to compassionate end-of-life and home health care.
Job Responsibilities
- Provide skilled nursing care to home health and hospice patients in accordance with physician orders and individualized care plans
- Monitor and document patient conditions, symptoms, and responses to treatment
- Administer prescribed medications and treatments safely and accurately
- Assist patients with disease management, symptom control, and comfort measures
- Educate patients and family members on medications, treatments, and ongoing care needs
- Collaborate with physicians, registered nurses, therapists, social workers, and other members of the interdisciplinary care team
- Observe and report changes in patient status promptly
- Maintain accurate and timely electronic medical documentation
- Promote patient safety while maintaining compliance with state and federal regulations
- Participate in care conferences and contribute to individualized patient care planning
- Deliver compassionate emotional support to patients and their families throughout the continuum of care
Education & Certification Requirements
- Current Licensed Practical Nurse (LPN) license in the State of Idaho or eligible for Idaho licensure
- Graduate of an accredited Practical Nursing program
- Minimum of 1 year of Home Health and Hospice nursing experience required
- Current BLS (Basic Life Support) certification
- Valid driver's license with reliable transportation and current auto insurance
- Ability to travel throughout assigned service areas
- Strong assessment, communication, and organizational skills
- Experience with electronic medical records preferred
