COMPANY PROFILE: 
This organization is a leading provider of integrated medical services, caring for individuals from post-hospitalization to “home” in various settings, such as skilled nursing facilities (SNFs).  They bring quality medical care to the geriatric population, as well as people with complex conditions, special needs, and intellectual and/or developmental disabilities.  Their providers make a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve.


POSITION DESCRIPTION: 
This nurse practitioner will provide primary care to a dedicated panel of geriatric patients, seeing them where they reside.  This includes assisted living facilities (ALFs), independent living facilities (ILFs), memory care units, and continuing care retirement communities (CCRCs).

A low number of patients are seen per day, allowing ample time to provide proper attention and support for the patients.

Overall duties of the role include, but are not limited to:

- plan, direct, and evaluate medical care
- examine and treat chronic illnesses
- examine and treat acute illnesses
- order and interpret laboratory and diagnostic testing
- collaborate with caregivers and other healthcare professionals
- provide referrals as necessary

The practice uses an EHR on a mobile-based platform and providers are supported by dedicated team members who assist with orders, referrals, etc.

This position will also partially serve as a clinical leader in the field, helping to grow the organization's excellent reputation.  The role has significant potential for upward mobility.

Providers share on-call responsibilities, which is phone triage only and light volume.  Providers are on call no more than 3 nights per month and flexibility in scheduling is offered.



REQUIREMENTS:
- Virginia NP license
- DEA
- 1+ year advanced practice experience required
- previous relevant experience required
