Synergy Care is seeking a compassionate and motivated Physical Therapist to join our team at Concordia Life Plan Community SNF/ Concordia Senior Living AL/IL in Oklahoma City, Oklahoma. This is a full-time opportunity working in a supportive AL/IL community with an on-site outpatient clinic and SNF, allowing you to build meaningful relationships with residents while providing high-quality, patient-centered care.

What You’ll Do

• Provide high-quality physical therapy services to residents in an AL/IL community and outpatient setting/SNF

• Develop individualized treatment plans that promote mobility, independence, and quality of life

• Collaborate with an interdisciplinary healthcare team to deliver exceptional care

• Build meaningful relationships with residents and their families

What We’re Looking For

Graduate of an accredited Physical Therapy programLicensed Physical Therapist or eligible for licensure in Oklahoma Passion for working with the geriatric population and improving functional outcomes
