Supervision

Reports to the Get Connected Program Director.

Schedule

Work hours are generally 8:30 am - 5:00 pm Monday through Friday, flexible based on work demands and in consultation with a supervisor, 40 hours per week. This is a full-time, exempt position. Some evenings and weekends are required for support group coverage.

Program Responsibilities

  • Manage the daily operational elements of the Get Connected (GC) Program in Franklin County in

collaboration with the Get Connected Team

o Assist and advocate for program participants by providing the following GC Care Navigation services:

o Timely information and referral services for program participants who are seeking behavioral

health services in Central Ohio

o Provide system navigation and community resource linkage to program participants who have

experienced difficulty connecting to care due to psychosocial or systemic barriers

o Serve as a 3rd party intermediary for program participants who need assistance navigating a

provider's client rights and grievance policy and process

o As a member of the GC Program Team, provide the following core program services on an as-needed basis:

o Answer the GC Program Care Navigation line, respond to incoming text messages, monitor the

GC Program email inbox, and handle incoming referrals

o Provide facilitation coverage for mental health support groups

o Assist with the maintenance and development of the GC Program's client database

  • Manage operational elements of GC Programming that specifically address the care needs of the older adult population

o Provide GC Care Navigation services to older adults age 60 and older

o Liaise with mental health providers, agencies, and related programs in the Central Ohio community that serve older adults to develop and maintain referral relationships

o Manage information for providers who serve older adults within the GC Program's provider referral directory

o Assist in the continuous development and updating of GC Program policies and procedures, in

collaboration with the program director and GC Program team, with a focus on serving the older adult population

o Collaborate with the GC Program Director on management of funding proposals, program evaluation, and data reporting related to older adults served by the program

  • Balance individual daily direct service workload with other job responsibilities in accordance with program policies and procedures
  • Consistently participate in agency and program events, groups, activities, and work collaboratively with agency staff
  • Other duties, within reason and scope of the position's primary duties, may be assigned after

consultation between the employee and supervisor

Salary

$48,000 - $53,000 per year. Salary commensurate with experience and other qualifications

Required Qualifications:

  • Education in social services or related fields
  • Experience working with diverse communities
  • Strong mediation and de-escalation skills
  • Strong verbal, written, and organizational skills
  • Comfortable working with individuals living with mental health and substance use disorders
  • Valid driver's license or means of transportation

Preferred Qualifications:

  • At least five years' experience working in the social services field serving the older adult population
  • At least five years' experience working in the mental health/SUD system in central Ohio
  • Experience with web-based databases, Microsoft Office 365 applications, and macOS
