Primary Duties and
Responsibilities:
We are seeking a full-time care coordinator to serve as part of our team on the Flourish Mobile Health Clinic. This person will provide trauma-informed, harm reduction-oriented care coordination services to vulnerable populations, including individuals affected by substance use disorders (SUD), homelessness, and limited access to healthcare. This position supports the clinic''s interdisciplinary care model by providing care coordination specific to healthcare referrals and healthcare access, including but not limited to coordinating behavioral, primary care, and specialty health referrals. In addition, this person will provide crisis intervention, connect patients with community resources, engage in community outreach and coordinate on-site experiences for social work and public health students.

Required EducationVocational or technical training or degree in Social Work or related area is required. Bachelor''s degree in Social Work is preferred.
Required Experience1-3 years experience working with mental health, addiction, medical, and/or vulnerable populations is required. 4-6 years experience is preferred.
The selected candidate will be required to complete a background check satisfactory to the University.
