Valley Presbyterian Hospital is seeking a detail-oriented and customer-focused HIM Physician Documentation Liaison to support the integrity, accuracy, and completeness of patient medical records. In this vital role, you will partner closely with physicians, clinical staff, and internal departments to ensure medical documentation meets regulatory, legal, and accreditation requirements while promoting timely completion of patient records.

What Youll Do
- Audit inpatient and outpatient medical records for completeness and accuracy.
- Monitor physician documentation deficiencies and communicate incomplete or delinquent records.
- Notify physicians regarding potential suspensions and disciplinary actions in accordance with Medical Staff Rules and Regulations.
- Analyze discharged patient records to identify documentation deficiencies.
- Resolve documentation deficiencies within the electronic health record (EHR).
- Perform quality assurance reviews to ensure accuracy before communicating with providers.
- Collaborate with physicians and departments to ensure timely completion of medical records.
- Process medical record merges and maintain patient identity integrity.
- Manage rejected physician orders and assign them to the appropriate provider in the EHR.
- Release confidential patient information in compliance with HIPAA and state and federal regulations.
- Complete birth certificates and assist with declarations of paternity when needed.
- Register and assist patients with the Hospital Patient Portal.
- Monitor electronic fax and email systems, including Forward Advantage.
- Respond promptly to incoming telephone calls and voicemail inquiries.
- Assist patients with medical record amendment requests in accordance with HIPAA requirements.
- Maintain knowledge of applicable regulations, compliance standards, and departmental policies.
- Participate in quality improvement and compliance initiatives.
- Perform additional duties as assigned.
What You Bring
- Previous medical records experience in an acute care hospital or coursework in Health Information Technology required.
- Strong organizational skills with exceptional attention to detail.
- Excellent written and verbal communication skills.
- Outstanding customer service and interpersonal skills.
- Ability to effectively prioritize and manage multiple responsibilities in a fast-paced environment.
- Intermediate knowledge of medical terminology preferred.
- Intermediate proficiency with Microsoft Outlook, Word, and Excel.
- Experience working with electronic health record systems preferred.
Education
- High school diploma or equivalent required.
- Some college coursework or an Associates degree preferred.
Licensure & Certifications
- Los Angeles City Fire Card required within 30 days of hire.
- Registered Health Information Technician (RHIT) credential or RHIT eligibility strongly preferred.
