Currently seeking an experienced LPN for one of our top home care agency partners with service locations in the Brockton/Abington/Whitman/to Weymouth/Braintree, MA areas.
- Full-time, Monday-Friday (8 hour days)
- No weekends, on-call, or holidays required.
- Excellent hourly pay rates and full benefits package available.
Role and responsibilities include but not limited to:
The Licensed Practical Nurse will provide direct patient care, and patient advocacy assistance in the coordination of care through communication with the multidisciplinary team and with the RN Case Manager.
Follows the nursing care plan, established by the case manager, based on nursing diagnoses that includes measurable, realistic, and time directed goals and nursing interventions specific to the problem and individual patient.
The LPN reports information about progress or changes to the primary nurse.
**Requires Minimum 1 year current/recent Home Care LPN experience**
