What You'll Do

As a Physical Therapist, you will play a vital role in delivering comprehensive, patient-centered care. Reporting to the Rehabilitation Team Lead, you will:

  • Perform comprehensive evaluations, assessments, and diagnoses

  • Develop and implement individualized care plans tailored to each patient

  • Provide hands-on therapeutic interventions in a home care setting

  • Educate patients and families to support long-term health and recovery

  • Collaborate with a multidisciplinary team, including specialty programs such as:

  • Parkinson's Disease care

  • COVID-19 recovery

  • Post-acute stroke rehabilitation

  • Respond to changing patient needs with clinical judgment and adaptability

What You'll Bring

Education

  • Bachelor's degree or higher from an accredited Physical Therapy program

Experience

  • At least 1 year of experience in acute care, rehabilitation, or a related setting
  • Home care experience preferred, but not required

Licensure & Certification

  • Active Massachusetts Physical Therapist (PT) license
  • BLS certification (required within 30 days of hire)
