Home Care Registered Nurse

We are seeking a compassionate and reliable Registered Nurse (RN) to join our home care team serving clients throughout the Fall River, MA area. This per diem position is ideal for a nurse looking for supplemental income and schedule flexibility while making a meaningful impact in the lives of clients receiving care at home.

The RN will primarily perform non-skilled nursing visits, including client assessments, admissions, reassessments, supervisory visits, caregiver oversight, and ongoing care plan management.

Compensation

Visits are pre-billed based on the estimated time required:

• Initial Assessments: 2.5 hours = $125 per visit

• Admissions: 2.0 hours = $100 per visit

• Other nursing visits compensated at the equivalent hourly rate of $50/hour

• Responsibilities

• Conduct comprehensive in-home client assessments

• Complete admission visits for new home care clients

• Perform reassessments and routine client evaluations

• Develop, review, and update individualized plans of care

• Conduct caregiver supervisory visits

• Evaluate client needs and monitor changes in condition

• Coordinate care with caregivers, families, physicians, and agency staff

• Ensure services are delivered in accordance with agency policies and state regulations

• Complete accurate and timely documentation

• Provide health and safety education to clients and families

• Participate in quality assurance and care coordination activities
