Home Health RN - Full Time Schedule: Monday - Friday Central Vermont Home Health & Hospice is more than just a healthcare provider; we're a community-focused, not-for-profit organization dedicated to enhancing the well-being of central Vermonters. The Home Health Travel RN provides skilled nursing care to patients in their homes, conducting assessments, developing and implementing care plans, coordinating with the interdisciplinary team, and ensuring timely, accurate documentation. This role supports quality patient outcomes through clinical expertise, effective communication, and efficient management of home health visits throughout the service area.
Shift: DAYS: Monday-Friday (with weekend rotation approximately every 4th week)
