Position Overview Southwoods Health is seeking a dynamic and qualified Home Health Clinical Manager. In this role, you will safely supervise and guide direct patient care in the home setting. Responsibilities include implementing plans of care, reviewing clinical documentation, and providing hands-on staff coaching and education. Essential Duties & Responsibilities - Clinical Leadership: Act as a clinical leader to actively support positive patient outcomes and exceptional care experiences.
- Care Plan Management: Review and optimize care plans to meet patient needs safely, effectively, and efficiently.
- Team Supervision: Manage a multidisciplinary team to ensure the accurate, safe, and compassionate delivery of home care.
- Quality & Documentation: Monitor clinical documentation, perform regular audits, and implement corrective action plans to fix deficiencies.
- Staff Development: Assist with the onboarding, orientation, and continuous training of new clinical staff members.
- Regulatory Compliance: Ensure all home health processes maintain strict compliance with relevant regulatory agencies (e.g., OSHA, ODH, BOP, TJC).
- Resource Optimization: Drive cost containment initiatives by utilizing facility resources appropriately.
- Confidentiality: Maintain absolute patient privacy and guarantee the strict confidentiality of all protected health information (PHI/HIPAA).
- Ad-Hoc Duties: Participate in facility training initiatives and perform other leadership duties as assigned.
Qualifications & Requirements - Licensure: Active Registered Nurse (RN) license in the State of Ohio.
- Certification: Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
- Education: Bachelor of Science in Nursing (BSN) is highly preferred.
- Clinical Skills: Advanced critical thinking, proactive problem-solving abilities, and a strong capacity to articulately document a patient's clinical picture.
- Leadership Skills: Proven leadership experience with the ability to delegate tasks appropriately to a multidisciplinary team.
- Communication: Exceptional verbal and written communication skills paired with a consistently professional demeanor.
- Workflow Management: Excellent prioritization, organization, and time management skills.
