Hiring: Home Health Intake Coordinator

Facility: Adventist Health Clear Lake
Department: Home Health Administration
Contract: 13 weeks
Pay Rate: $39-44/hour
Schedule: Monday Friday, Day Shift
Working Hours: 8:00 AM 4:30 PM

Job Description:

We are seeking an experienced Intake Coordinator to process home health referrals, coordinate patient admissions, communicate with physicians and healthcare teams, and ensure patients are scheduled for appropriate services. Responsibilities include verifying insurance benefits, maintaining patient records, answering calls, and coordinating referrals with clinical staff.

Requirements:

  • At least 1.5 years of relevant experience.

  • Active RN, LVN, or LPN license.

  • CPR certification required.

  • Epic experience required.

  • Associate degree in Nursing or equivalent education/experience required.
