We invite you to join our esteemed healthcare team as a Occupational Therapist, where you will play an integral role in enhancing the independence and quality of life of our geriatric patients through care delivered in the comfort of their homes. This position offers scheduling flexibility and operates out of Monmouth and Middlesex Counties.

We collaborate closely with physicians, specialists, and care teams to ensure coordinated, patient-centered treatment, allowing our clinicians to practice without the burnout commonly associated with high-volume clinical settings.
Responsibilities
Deliver individualized, one-on-one occupational therapy services in patients' homes
Conduct comprehensive evaluations and develop tailored plans of care
Maintain professional communication and collaboration with referring physicians, specialists, and care teams
Cultivate strong, trust-based relationships with patients and their families to support optimal clinical outcomes
Coordinate care effectively to ensure continuity between patients and providers
Complete all clinical documentation with accuracy and in a timely manner
Represent Elite Mobility Home Therapy with the highest standards of professionalism and compassion
Who You Are
A licensed Occupational Therapist in good standing
Possess strong communication and interpersonal skills
Capable of working independently and effectively in a home-based care setting
Committed to a patient-focused, relationship-driven approach to care
Able to work a minimum of 2 non-consecutive days a week seeing at least 1 patient
A valid driver's license and access to a reliable vehicle
Home health experience is advantageous, though not required
