Home Health Occupational Therapy

Job Description:

We're seeking a dedicated and compassionate Occupational Therapist (OT) to join our care team. The Occupational Therapist will assess patient needs and develop individualized treatment plans that promote functional independence in daily living activities. This role is ideal for an OT who is passionate about helping patients improve their quality of life in a supportive and personalized care environment.

Key Responsibilities:

• Perform comprehensive occupational therapy evaluations in the patients home or clinical setting
• Develop, implement, and revise individualized treatment plans based on patient goals and physician directives
• Assist patients with regaining or enhancing the skills needed for daily living and working
• Educate patients and caregivers on adaptive equipment and home safety modifications
• Monitor and document patient progress toward therapy goals
• Collaborate with physicians, nurses, physical therapists, and other members of the healthcare team
• Ensure therapy services are delivered in compliance with federal, state, and agency policies
• Degree in Occupational Therapy from an accredited program
• Current Occupational Therapy license in the state of Maryland
• Minimum of 1 year of experience in occupational therapy (home health or rehab setting preferred)
• Current CPR/BLS certification
• Valid drivers license and reliable transportation (for home health roles)
• Excellent interpersonal, time management, and communication skills

Qualifications:

• Degree in Occupational Therapy from an accredited program
• Current Occupational Therapy license in the state of Maryland
• Minimum of 1 year of experience in occupational therapy (home health or rehab setting preferred)
• Current CPR/BLS certification
• Valid drivers license and reliable transportation (for home health roles)
• Excellent interpersonal, time management, and communication skills
