    About the Role:
    Join PACIFIC HEALTH SERVICES INC as a Home Health RN and make a meaningful impact in the lives of patients in Apple Valley, CA. We are looking for compassionate and skilled nurses to deliver exceptional care in a home setting, ensuring comfort and support for our clients and their families.

    Day Shift: Mon-Friday 12pm-8pm

    Responsibilities:
    • Conduct comprehensive assessments of patients' health and care needs.
    • Develop and implement personalized care plans in collaboration with healthcare teams.
    • Administer medications and treatments as prescribed by physicians.
    • Monitor and document patient progress and respond to changes in condition.
    • Educate patients and families on health management and home care techniques.
    • Coordinate with interdisciplinary teams to ensure holistic patient care.
    • Maintain accurate medical records and adhere to compliance regulations.
    • Provide emotional support and guidance to patients and their families.
    Requirements:
    • Valid RN license in the state of California.
    • Minimum of 1-2 years of nursing experience, preferably in home health or community settings.
    • Strong assessment, critical thinking, and communication skills.
    • Ability to work independently and manage time effectively.
    • Current CPR and First Aid certification.
    • Compassionate demeanor with a patient-centered approach to care.
    • Proficient in electronic medical records (EMR) systems.
    • Willingness to travel to patients' homes within the service area.
