Per Diem Physical Therapist to provide top-quality care to our patients in the
Chula Vista area and surrounding communities in San Diego County.

Setting: Home Health
Area: Chula Vista, CA (and nearby South Bay/San Diego County)
Pay: Competitive Per Diem Rate (based on experience and visit type)
Key Responsibilities
- Perform comprehensive physical therapy evaluations to assess patient condition, functional abilities, and home environment.
- Develop and implement individualized Plans of Care (POCs) in collaboration with the physician and interdisciplinary team (RNs, OTs, SLPs).
- Provide skilled physical therapy treatments, including therapeutic exercise, gait training, and instruction on the use of assistive devices.
- Educate patients and caregivers on safe and effective home exercise programs and disease management.
- Timely and accurate completion of all required documentation using an Electronic Medical Record (EMR) system.
- Adhere to all state and federal regulations for home health care.
Qualifications
- Current and unrestricted Physical Therapist (PT) license in the State of California.
- Doctor of Physical Therapy (DPT) degree preferred.
- Minimum of 1 year of professional experience as a Physical Therapist (home health experience is a plus!).
- Current CPR certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Excellent clinical, communication, and time-management skills.
