Millennium Home Care, a leading provider of in-home healthcare services, is looking for a dedicated full-time Physical Therapist to join our team.     Responsibilities:

    • Patient Assessment: Conduct comprehensive evaluations of patients' physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans.
    • Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients' mobility, strength, balance, and pain management.
    • Functional Training: Assist patients in developing compensatory strategies, adaptive techniques, and assistive device training to enhance their independence in activities of daily living.
    • Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long-term health and well-being.
    • Collaboration: Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
    • Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
    • Compliance: Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.

    Requirements:

    • Licensure: Current and valid Physical Therapist license in the state of Florida.
    • Experience: Minimum 6 months of experience in a hospital, skilled nursing facility, or rehab is requested.
    • Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.
    • Professionalism: Strong work ethic, reliability, and a commitment to providing high-quality care.
    • Transportation: Reliable transportation to visit patients in their homes.
