What You'll Do

• Complete in-home physical therapy evaluations

• Develop individualized treatment plans based on each patient's needs and goals

• Provide therapeutic exercise, gait training, balance training, mobility instruction, and other skilled interventions

• Help patients improve strength, function, endurance, and safety

• Educate patients and caregivers on home exercise programs and fall prevention

• Monitor patient progress and adjust treatment plans as appropriate

• Collaborate with nurses, physicians, therapists, and other members of the care team

• Complete accurate and timely documentation

What You'll Bring

• Degree from an accredited Physical Therapy program

• Active Arizona Physical Therapist license

• Current CPR/BLS certification

• Home health experience preferred

• Excellent communication and time-management skills

• Ability to work independently in a home health environment

• Reliable transportation and valid driver's license
