We’re hiring a Home Health Physical Therapist (PT) in Northside Jacksonville who wants schedule flexibility, strong earning potential, and the autonomy to manage their own day.

You’ll step into a home health role providing one-on-one Physical Therapy services throughout an established Northside Jacksonville territory. You’ll manage evaluations, reassessments, discharges, and ongoing treatment while building a schedule that works for you.

• You hold a clear and active Florida Physical Therapist license
• You’re comfortable providing independent, one-on-one patient care in the home
• You value autonomy, flexibility, and accountability
• Home health experience is a plus but not required
• Experienced Physical Therapists from other clinical settings are welcome

Compensation & Benefits

• Annual earning potential: $85,000–$120,000 based on productivity
• Productivity-based per-unit compensation
• Health insurance: Employer pays 50%
• Dental & vision insurance: 100% employer-paid
• Retirement: SIMPLE IRA with a 3% employer match after 1 year
• PTO: 10 days in Year 1, increasing to 11 days in Year 2
• $400 license reimbursement after 1 year
