Position Summary

We are seeking a compassionate and skilled Physical Therapist (PT) to join our growing home health team in Rock Springs, WY. In this role, you’ll provide one-on-one, patient-centered care in the comfort of patients’ homes—helping them regain independence and improve quality of life.

What You’ll Do

• Evaluate and develop individualized care plans for home health patients
• Deliver evidence-based physical therapy interventions
• Educate patients and caregivers on treatment plans and progress
• Collaborate with an interdisciplinary team to ensure optimal patient outcomes
• Maintain accurate and timely clinical documentation
• Adapt care based on patient progress and needs

Qualifications

• Doctorate or Master’s degree in Physical Therapy
• Current PT license in the state of Wyoming (or ability to obtain)
• Strong clinical, communication, and organizational skills
• Home health experience preferred, but not required
• Valid driver’s license and reliable transportation
