Physical Therapist (PT) - Full Time

Inspired Home Health Care Agency is rapidly expanding, and we are hiring immediately!

We are currently seeking a dedicated and compassionate Full-Time Physical Therapist (PT) to join our growing home health team. The Physical Therapist will serve as a patient's partner throughout the journey of restoring mobility, improving function, and maximizing overall quality of life.

Our therapists play a critical role in helping patients reach their highest level of independence through promotion, prevention, diagnosis, treatment, and rehabilitation.

Compensation

Salary: OR Pay-Per-Visit Rate Available

Key Responsibilities

Patient Assessment & Evaluation

  • Conduct initial OASIS evaluations and in-home assessments to determine physical therapy needs

  • Review patient medical histories, diagnoses, and physician orders

  • Identify patient-specific goals and develop individualized treatment plans

Treatment Planning & Implementation

  • Develop and implement treatment plans to restore mobility, strength, and functionality

  • Utilize various therapy techniques including therapeutic exercise, manual therapy, and adaptive equipment

  • Provide cost-effective treatments that improve motion, mobility, and independence

  • Reduce reliance on medications and provide non-surgical treatment alternatives when appropriate

Patient Education

  • Educate patients and caregivers on exercises, recovery techniques, and safety measures

  • Provide guidance on proper body mechanics and injury prevention

  • Develop home exercise and wellness programs tailored to each patient's needs

Progress Monitoring

  • Monitor patient progress and adjust treatment plans as necessary

  • Communicate regularly with physicians and healthcare team members regarding patient progress

Collaboration

  • Work closely with nurses, occupational therapists, physicians, and other healthcare professionals

  • Participate in interdisciplinary care coordination to ensure comprehensive patient care

Documentation & Compliance

  • Maintain accurate and timely patient documentation

  • Ensure compliance with HIPAA regulations and agency policies

  • Complete all documentation required for billing and insurance purposes

Qualifications

  • Active Arizona Physical Therapist (PT) License

  • Home Health experience preferred

  • Strong clinical assessment and communication skills

  • Ability to work independently in a field-based environment

  • Passion for delivering patient-centered care

Travel

This position requires travel to patient homes throughout assigned territories and the ability to manage a mobile caseload.
