What You'll Be Doing

As a Home Health Physical Therapist, you will evaluate patients in their homes and develop treatment plans designed to improve mobility, strength, balance, safety, and overall function.

Your responsibilities will include:

• Performing physical therapy evaluations and reassessments

• Identifying mobility limitations and functional deficits

• Creating individualized treatment plans based on patient needs and goals

• Implementing interventions focused on strength, balance, gait training, and fall prevention

• Teaching patients and caregivers exercises and safety techniques

• Monitoring progress and modifying treatment plans as appropriate

• Coordinating care with physicians, nurses, caregivers, and other healthcare professionals

• Completing documentation in accordance with Medicare home health requirements

What We're Looking For

Successful candidates typically possess:

• Current Hawaii Physical Therapist license

• Minimum one year of clinical experience

• Home health experience preferred but not required

• Strong communication and patient education skills

• Ability to work independently and manage a field-based schedule

• Reliable transportation and valid driver's license

• Commitment to providing compassionate, patient-centered care
