Our nurses receive strong clinical leadership, clear communication, and caseloads designed to minimize travel and maximize patient care.Service AreasThis position covers the North and Northwest Chicagoland suburbs, including: Arlington Heights, Palatine, Mount Prospect, Des Plaines, Park Ridge, Glenview, Northbrook, Wheeling, Buffalo Grove, Schaumburg, Hoffman Estates, and surrounding communities.What You'll DoProvide skilled nursing visits including wound care, medication management, post‑acute care, and patient education.Complete OASIS assessments (SOC, ROC, Recert, Discharge) within required timelines.Develop and update individualized plans of care in collaboration with physicians and interdisciplinary team members.Assess home environments for safety risks and document findings.Educate patients and caregivers on disease processes, medications, and symptom recognition.Communicate changes in condition promptly and obtain updated orders as needed.Document all care accurately and timely in the EMR.Coordinate care with PT/OT/ST and the broader clinical team.Manage a flexible caseload within assigned territories. RequirementsActive Illinois RN license.Minimum 1 year of nursing experience (home health preferred).Strong clinical judgment and ability to work independently.Reliable transportation.Experience with OASIS preferred.Familiarity with North/Northwest suburban communities is a plus.
