About the Role:Care Plus Hospice Care LLC is looking for a compassionate and dedicated Hospice CNA to join our team in the Phoenix Arizona Valley. Responsibilities:- Provide personal care services including bathing, grooming, oral hygiene, and dressing for hospice patients
- Assist with mobility, transfers, and positioning to ensure patient comfort and safety
- Monitor and report changes in patient condition, vital signs, and overall well-being to the supervising nurse
- Deliver compassionate emotional support to patients and their families throughout the end-of-life journey
- Assist with light housekeeping, meal preparation, and other activities of daily living (ADLs)
- Document patient care activities accurately and in compliance with hospice care standards
- Collaborate with the interdisciplinary hospice care team to ensure a holistic, patient-centered approach
Requirements:- Active Certified Nursing Assistant (CNA) license in the state of Arizona
- Prior experience in hospice, palliative care, or home health preferred
- Strong understanding of end-of-life care, comfort measures, and patient dignity
- Excellent communication and interpersonal skills with patients and families
- Reliable transportation and ability to travel to patient homes in the Scottsdale, AZ area
- Ability to work independently with compassion, patience, and professionalism
- Current CPR certification and ability to pass a background check
