    Phoenix Home Care is currently seeking a Hospice Director of Nursing in Independence!

Our patients are facing end-of-life health care needs and we are seeking a Director of Clinical Services who will be responsible for the overall direction of hospice clinical services with dignity, compassion & a warm heart.

    POSITION SUMMARY

    The Director of Clinical Services is responsible for the overall direction of hospice clinical services. Maintains the quality of patient care in accordance with the Agency's objectives and policies, through planning, coordination, implementation and evaluation of the hospice services provided.

    DUTIES AND RESPONSIBILITIES

    • Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
    • Provide supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards.
    • Perform chart audits.
    • Gather data for QAPI and participates in QAPI activities.
    • Apprise clinical staff performance during their probationary period and annually, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.
    • Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.
    • Assist with the orientation of new staff.
    • See patients in the home setting as needed.
    • Participate in audits, IDG meetings & QAPI.
    • Maintains adequate and appropriate Medical Supplies.
    • Responsible for orientation of new personnel as well as in-services for current staff.
    • Responsible for ensuring Medicare, State, and CHAP regulations are followed by all clinical staff.
    • Assume responsibility for self-development by continually striving to improve his/her Nursing/Management practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.

    QUALIFICATIONS

    • Must be a graduate of an accredited School of Nursing.
    • Must be a Registered Nurse licensed by the state.
    • Experience: Must have a minimum of 2 years nursing experience in home hospice or skilled home care. (hospice preferred)
    • Ability to market and deal tactfully with customers and the community.
    • Must have excellent assessment skills and problem-solving skills.
    • The ability to communicate well, both verbally and in writing.
    • Must have the ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.

    Phoenix provides Home Health Care services to those who desire to remain independent at home. Phoenix provides acute Skilled Nursing through Home Health Services; Private Duty Nursing to Children with Special Needs; Privately Paid Services such as nursing, nurse-aide or companion level assistance with activities of daily living from a few hours a day to around-the-clock care; State funded In-home Services (homemaker chores, errands and shopping, personal care, education, medication set-ups and reminders; and Consumer Directed Services where Phoenix serves as a state vendor to provide administrative support services.
