About the Role:
Care Plus Hospice Care LLC is looking for a compassionate and driven Hospice Liaison / Care Coordinator to join our dedicated team in Scottsdale, AZ. In this meaningful role, you'll serve as a vital bridge between patients, families, and healthcare providers — ensuring seamless transitions into hospice care. Responsibilities:

• Build and maintain relationships with hospitals, skilled nursing facilities, physicians, and other referral sources to drive hospice admissions
• Educate healthcare professionals, patients, and families on hospice eligibility, services, and the benefits of end-of-life care
• Assist with the coordination of timely hospice evaluations and admissions
• Collaborate with the interdisciplinary care team to ensure smooth patient transitions and continuity of care
• Serve as a community ambassador, representing Care Plus Hospice Care at outreach events and healthcare networking opportunities
• Maintain accurate documentation of referral activity, patient interactions, and care coordination efforts
• Provide ongoing support and communication to families navigating the hospice process

Requirements:

• Prior experience in hospice, palliative care, home health, or a related healthcare setting strongly preferred
• Proven background in healthcare sales, community outreach, or clinical liaison roles
• Strong knowledge of hospice regulations, Medicare/Medicaid eligibility criteria, and end-of-life care principles
• Excellent interpersonal and communication skills with a compassionate, patient-centered approach
• Self-motivated, organized, and able to manage a territory independently
• Valid driver's license and reliable transportation required for local travel throughout the Scottsdale area
