Description
General Definition and Scope of Job
Participates in a variety of activities designed to facilitate the continuity of the delivery of care to patients and families. Assists patients and their families to cope with the personal, interpersonal and practical problems that arise at end of life. These activities will be achieved through consultation and coordination efforts with patients, families, caregivers and the Hospice team.
Minimum Skills, Experience and Educational Requirements
Masters in Social Work or a Bachelors in Social Work functioning under the supervision of an MSW, CSW, with appropriate State of Michigan Social Work Registration
One year of Social work experience or after hire 1 year of supervision by a CSW
Knowledge of and agreement with Hospice concepts and philosophy
Excellent written and verbal communication skills, interpersonal skills and public speaking
Flexible to meet the needs of all persons serviced by Hospice
Current Michigan Driver’s License
Critical Demands of the Job
Occasional prolonged periods of sitting, driving
Occasional prolonged periods of data input into the computer system
Frequent walking to respond to department needs
Occasional moving of office / paper supplies up to 40 pounds
Ability to stair climb minimum of 2 flights
Ability to work in less than ideal situation/unpredictable environment
