We are seeking an experienced and compassionate Hospice PRN Case Manager who is dedicated to providing exceptional end-of-life care with dignity, empathy, and clinical excellence, and who is willing to travel in DFW Area

This role is ideal for a nurse who is passionate about providing high-quality, patient-centered end-of-life care while enjoying flexibility in scheduling. You will work closely with patients, families, and an interdisciplinary team to ensure comfort, dignity, and support throughout the hospice journey.


Service Areas:
  •  Dallas 
  •  Duncanville 
  •  Mansfield 
  •  McKinney 
  •  Allen 
Key Responsibilities:
  •  Conduct comprehensive patient assessments and develop individualized care plans 
  •  Provide skilled nursing care and symptom management 
  •  Coordinate care with physicians, social workers, chaplains, and other team members 
  •  Educate patients and families on disease progression, medications, and care techniques 
  •  Monitor patient condition and adjust care plans as needed 
  •  Maintain accurate and timely clinical documentation 
  •  Participate in on-call rotation as needed (PRN basis) 
  •  Ensure compliance with hospice regulations and standards 
Qualifications:
  •  Current and unrestricted RN license in the state of Texas
  •  Minimum 1–2 years of hospice, home health, or related experience preferred
  •  Strong clinical assessment and critical thinking skills 
  •  Excellent communication and interpersonal abilities 
  •  Ability to work independently and manage time effectively 
  •  Reliable transportation and valid driver’s license 
  •  Current CPR certification (required)
