Our hospice and palliative care programs are available across 16 counties in North Texas, including: Collin, Cooke, Dallas, Denton, Ellis, Fannin, Grayson, Henderson, Hunt, Johnson, Kaufman, Rockwall, Tarrant, Van Zandt, and parts of Wise and Navarro Counties. Primary functions are to administer nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assume the responsibility for coordination of care.
