Hospice Registered Nurse

• Coordinates hospice care by workingwith other team members as well as with the patients/family members to ensureneeds are met.
• Evaluate the effectiveness ofhospice services to the patient and family on an ongoing basis.
• Perform evaluations for hospice carealong with informational visits as needed.
• Utilize the nursing process toprovide professional nursing care.
• Consult with the attendingphysician and Hospice Medical Director concerning changes in patients’condition requiring order changes and/or changes in the plan of care.
• Coordinate patient services withother members of the IDG as well as contracted services, when needed.
• Submit complete, accurate andrelevant clinical notes in a timely manner.
• Submit patient visit scheduleweekly.
• Participate in IDG and caseconferences.
• Provide guidance and supervision tothe LPN and supervises the LPN a minimum of every 30 days. Cooperate with otheragencies providing nursing or related services to provide continuity of careand to implement a comprehensive care plan.
• Participate in staff developmentmeetings.
• Prepare the care plan for theHospice Aide/Home Attendant.

Requirements

• Must be a graduate from an accredited School of Nursing.
• Must be licensed in the State of Virginia as a RegisteredNurse.
• A minimum of two (2) years of hospice/home care experience;at least one (1) year of supervisory experience.
• Must have excellent assessment skills and problem-solvingskills.
• Should be skillful in organization and in the principles oftime management and have knowledge of case management processes.
• Must be a licensed driver with an insured automobile in goodworking order.
• The ability to communicate well, both verbally and inwriting.
