Job Summary
We are seeking a dedicated and experienced Registered Nurse Case Manager to join our Hospice team. The ideal candidate will play a vital role in coordinating patient care, ensuring seamless transitions across various care settings, and optimizing health outcomes. This position offers an opportunity to provide palliative care, utilizing extensive nursing expertise to support patients through complex medical journeys. Duties

• Conduct comprehensive patient assessments, including vital signs and physiological evaluations, to determine care needs.
• Develop and implement individualized care plans in collaboration with multidisciplinary teams, focusing on palliative needs.
• Coordinate with hospital discharge planners to ensure continuity of care.
• Collaborate with physicians, and other healthcare professionals in managing complex cases involving geriatric and end-of-life care.
• Provide ongoing patient/family education on treatment plans, medication management, and hospice care strategies.
• Ensure compliance with hospice policies, safety standards, and regulatory requirements including HIPAA and medical terminology standards.

Qualifications

• Valid RN license with experience in home care patients.
• Demonstrated expertise in case management is advantageous.
• Strong knowledge of physiology, medical terminology, and patient assessment techniques.
• Experience working with diverse patient populations including geriatrics, behavioral health patients, or those requiring infectious disease management.
• Familiarity with Hospice MD EMR, Tiger Connect and healthcare regulations such as HIPAA is essential.
• Excellent communication skills with the ability to collaborate effectively across multidisciplinary teams and provide exemplary patient service. Responsibilities 

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans 
• Administer medications and monitor patients for side effects or reactions  
• Document any changes in patient conditions and report on changes 
• Maintain accurate records of patient care and discuss observations with the supervisor  

Qualifications

• Currently licensed Registered Nurse (RN) in California
• Previous experience as a Registered Nurse is preferred
• Excellent interpersonal and client care skills
• Valid driver’s license 
• First Aid/CPR certification required 
• Bilingual Preferred
