They are seeking a talented Hospice Registered Nurse (RN) for 37.5 hours per week.

Position Summary:

The Hospice Registered Nurses (RN) overall responsibility is to provide direct client care and case management to an assigned caseload in the home setting in accordance with physician’s orders.  This includes admission of clients for service and determining and developing a plan of care.  The Hospice Registered Nurse (RN) assures completeness, timeliness and appropriateness of the patient care plan, its implementation and ongoing evaluation. Assures communication with the multidisciplinary team, support services and community resources to ensure that care goals are achieved in the most efficient and cost-effective manner possible. Ensures that documentation is timely, accurate, complete, and reimbursable.

Qualifications:

• Hospice experience (preferred)
• 1 year of med/surg, ER, or home health experience if no previous Hospice experience (required)
• Licensure: State of New Hampshire or Compact State Nursing License
• Minimum ADN preparation
• NH Driver’s License and current automobile and liability insurance
• Tech savvy (preferred)
• Strong documentation, communication, and symptom management skills (required)
• Must be flexible and family oriented
• Current BLS
• Comfortable with death and terminal illness

Schedule:

• Monday -Friday; 8a-4:30p (37.5 hour per week)
• Occasional weekend and some back up on-call requirements (1 weekend per month)

Compensation:

•  $35-$50 Per/Hour
• On-call and Call-back compensated
• Mileage reimbursement at IRS rate

Work Location:

• Out in the field providing patient visits (homes and facilities)

Coverage Area:

• Southern Lakes: Franklin, Northfield, Sanbornton, Gilmanton, Loudon

Productivity Requirements:

• 4-5 visits per day (depending on care level needed)
